ABSTRACT

ABSTRACT The objective of this preliminary study was to correlate diffusion tensor imaging (DTI) alterations with the cognitive profile of patients with amyotrophic lateral sclerosis (ALS). Methods This was a case-control study conducted from December 1, 2012 to December 1, 2014. Clinical and demographic data were recorded. A neuropsychological test battery adapted to ALS patients was used. An MRI with DTI was performed in all patients and fractional anisotropy (FA) was analyzed in the white matter using the tract based spatial statistics program. Results Twenty-four patients with ALS (15 females, mean age 66.9 + -2.3) and 13 healthy controls (four females, average age 66.9 + - 2) were included. The DTI showed white matter damage in ALS patients vs. healthy controls (p < 0.001). Discussion In our preliminary study the alterations of white matter in DTI were significantly associated with cognitive impairment in patients with ALS.


RESUMEN El objetivo del presente estudio preliminar fue correlacionar alteraciones del Tensor de Difusión (TD) con el perfil cognitivo de pacientes con Esclerosis Lateral Amiotrofica (ELA). Metodos Se realizó estudio casos-controles entre el 1 de Diciembre del 2012 hasta el 1 de Diciembre del 2014. Se registraron datos clínicos y demográficos. Se utilizó batería de tests neuropsicológicos adaptada a ELA. Se realizó RMN de cerebro con TD en todos los pacientes, la Fracción de Anisotropía (FA) se analizó en sustancia blanca, utilizando el programa Tract Based Spatial Statistics. Resultados Se incluyeron 24 pacientes con ELA (15 mujeres, edad media 66.9 + -2.3) y 13 controles sanos (4 mujeres, edad media 66.9 +-2). El TD mostró daño en sustancia blanca en los pacientes con ELA vs controles (p < 0.001). Discusión En nuestro estudio preliminar las alteraciones de sustancia blanca en TD se asociaron significativamente con alteraciones cognitivas en pacientes con ELA.

ABSTRACT

ABSTRACT OBJECTIVE To describe the methodology used in the process of setting health priorities for community intervention in a community of older adults. METHODS Based on the results of a health diagnosis related to active aging, a prioritization process was conceived to select the priority intervention problem. The process comprised four successive phases of problem analysis and classification: (1) grouping by level of similarity, (2) classification according to epidemiological criteria, (3) ordering by experts, and (4) application of the Hanlon method. These stages combined, in an integrated manner, the views of health team professionals, community nursing and gerontology experts, and the actual community. RESULTS The first stage grouped the identified problems by level of similarity, comprising a body of 19 issues for analysis. In the second stage these problems were classified by the health team members by epidemiological criteria (size, vulnerability, and transcendence). The nine most relevant problems resulting from the second stage of the process were submitted to expert analysis and the five most pertinent problems were selected. The last step identified the priority issue for intervention in this specific community with the participation of formal and informal community leaders: Low Social Interaction in Community Participation. CONCLUSIONS The prioritization process is a key step in health planning, enabling the identification of priority problems to intervene in a given community at a given time. There are no default formulas for selecting priority issues. It is up to each community intervention team to define its own process with different methods/techniques that allow the identification of and intervention in needs classified as priority by the community.


RESUMO OBJETIVO Descrever a metodologia utilizada no processo de estabelecimento de prioridades em saúde para intervenção comunitária, numa comunidade idosa. MÉTODOS Partindo dos resultados de um diagnóstico de saúde no âmbito da promoção do envelhecimento ativo, concebeu-se um processo de estabelecimento de prioridades a fim de selecionar o problema prioritário para intervenção. O processo integrou quatro etapas sucessivas de análise e classificação dos problemas: (1) agrupamento por nível de similitude, (2) classificação de acordo com critérios epidemiológicos, (3) ordenação por peritos e (4) aplicação do método de Hanlon. No decurso destas etapas, combinaram-se, de forma integrada, as perspetivas dos profissionais da equipe de saúde, de peritos em enfermagem comunitária e gerontologia e da própria comunidade. RESULTADOS Na primeira etapa, agruparam-se por nível de similitude os problemas identificados, constituindo-se um corpo de 19 problemas para análise. Na segunda, esses problemas foram classificados pelos elementos da equipe de saúde, mediante a aplicação de critérios de cariz epidemiológico (magnitude, vulnerabilidade e transcendência). Os nove problemas mais relevantes resultantes da operacionalização da segunda etapa do processo foram submetidos a análise por peritos, e selecionados os cinco problemas com maior pertinência de atuação. Na última etapa, com recurso à participação de líderes formais e informais da comunidade, identificou-se o problema prioritário para intervenção nessa comunidade específica: a Baixa Interação Social na Participação Comunitária. CONCLUSÕES O processo de estabelecimento de prioridades é uma etapa fundamental do planejamento em saúde, permitindo identificar os problemas prioritários a intervir numa determinada comunidade e num determinado momento. Não existem fórmulas predeterminadas para a seleção de problemas prioritários. Cabe a cada equipe de intervenção comunitária a definição de um processo próprio com diferentes métodos/técnicas que possibilitem a identificação e intervenção em necessidades classificadas como prioritárias pela comunidade.

ABSTRACT

O objetivo dessa pesquisa é comparar os elementos associados à participação coletiva de mulheres indígenas pertencentes a duas organizações de direitos humanos, Kalli Luz Marina (México) e Federação Regional de Mulheres Asháninkas, Nomatsiguengas e Kakintes da Selva Central (Peru), com base na perspectiva de gênero. Efetuou-se uma análise qualitativa por entrevistas semiestruturadas realizadas com as participantes, além de investigação dos arquivos das organizações. Verificou-se que a organização das mulheres indígenas é motivada pela percepção de injustiça, a capacidade de agência, identidade coletiva e emoções presentes no processo, o que as leva a mobilizar-se e a manter-se em seus grupos, com uma participação ativa, buscando relações de gênero equitativas. As organizações de mulheres nahuas, asháninkas e nomatsiguengas contribuem para a criação de espaços de encontro para seus pares e fornecem elementos relevantes para o desenvolvimento de atividades de gênero culturalmente situadas


The objective of this investigation is to compare the elements associated with the collective participation of indigenous women from two human rights organizations, Kalli Luz Marina (Mexico), and the Regional Federation of Women Asháninkas, Nomatsiguengas and Kakintes, from the Selva Central (Peru), with perspective to gender. Using the Theory of collective action framework, a comparative qualitative analysis was implemented, collecting information through semi-structured interviews of the participants, and organization's archival research. It was found that the organizational process of indigenous women is motivated by the perception of injustice, agency capacity, collective identity and emotions present in the process, which leads them to mobilize and remain in their groups with active participation looking for equal relationships between the genders. Nahua, Asháninka and Nomatsiguenga women organizations contribute to the creation of meeting spaces for their peers and provide relevant elements for the development of culturally situated gender agendas


El objetivo de la investigación es comparar los elementos asociados a la participación colectiva de mujeres indígenas de dos organizaciones de derechos humanos, desde la perspectiva de género. Se efectuó un análisis comparativo de corte cualitativo de los marcos de acción colectiva de mujeres indígenas pertenecientes al Kalli Luz Marina (México) y la Federación Regional de Mujeres Asháninkas, Nomatsiguengas y Kakintes de la Selva Central (Perú), a través de entrevistas semiestructuradas realizadas a las participantes e investigación de archivos de las organizaciones. Se encontró que el proceso organizativo de mujeres nahuas, asháninkas y nomatsiguengas está motivado por la percepción de injusticia, la capacidad de agencia, la identidad colectiva y las emociones presentes en el proceso, que las lleva a movilizarse y mantenerse en sus colectivos con una participación activa, buscando relaciones de género equitativas. Las organizaciones de mujeres indígenas crean espacios de encuentro y desarrollan agendas de género culturalmente situadas

ABSTRACT

OBJECTIVE: We aimed to evaluate the prognostic and predictive value of the nucleotide excision repair-related gene GTF2H5, which is localized at the 6q24.2-26 deletion previously reported by our group to predict longer survival of high-grade serous ovarian cancer patients. METHODS: In order to test if protein levels of GTF2H5 are associated with patients' outcome, we performed GTF2H5 immunohistochemical staining in 139 high-grade serous ovarian carcinomas included in tissue microarrays. Upon stratification of cases into high- and low-GTF2H5 staining categories (> and < or = median staining, respectively) Kaplan-Meier and log-rank test were used to estimate patients' survival and assess statistical differences. We also evaluated the association of GTF2H5 with survival at the transcriptional level by using the on-line Kaplan-Meier plotter tool, which includes gene expression and survival data of 855 high-grade serous ovarian cancer patients from 13 different datasets. Finally, we determined whether stable short hairpin RNA-mediated GTF2H5 downregulation modulates cisplatin sensitivity in the SKOV3 and COV504 cell lines by using cytotoxicity assays. RESULTS: Low expression of GTF2H5 was associated with longer 5-year survival of patients at the protein (hazard ratio [HR], 0.52; 95% CI, 0.29 to 0.93; p=0.024) and transcriptional level (HR, 0.80; 95% CI, 0.65 to 0.97; p=0.023) in high-grade serous ovarian cancer patients. We confirmed the association with 5-year overall survival (HR, 0.55; 95% CI, 0.38 to 0.78; p=0.0007) and also found an association with progression-free survival (HR, 0.72; 95% CI, 0.54 to 0.96; p=0.026) in a homogenous group of 388 high-stage (stages III-IV using the International Federation of Gynecology and Obstetrics staging system), optimally debulked high-grade serous ovarian cancer patients. GTF2H5-silencing induced a decrease of the half maximal inhibitory concentration upon cisplatin treatment in GTF2H5-silenced ovarian cancer cells. CONCLUSION: Low levels of GTF2H5 are associated with enhanced prognosis in high-grade serous ovarian cancer patients and may contribute to cisplatin sensitization.
